Skimmed over your video. Main things, you are wearing really bad shoes for squatting. Go barefoot over those. You claim you know what proper form is and are getting really irate at lawler yet he tries to give you proper advice.
You have really poor hamstring / hip flexibility, there is no tightness in your back at all ( note the buttwink ). You are also breaking at the knees as opposed to the hips, look into hip drive.
These are the very basics of squatting.
