4. WOL, WMO, WWS, etc. links and/or other clear indication of your performance in a raid:
I really don't have logs since I went casual a week into Cata. But I will explain guild experience down below, and hopefully that sells myself a trial opportunity to your guild.
5. A history of your previous guild(s)/server(s), their progression, your rank and responsibilities in the guild, and reason(s) for leaving, also include any references we may contact:
Alright. Vanilla I was a baddie. Didn't do much, yatta, yatta. During The Burning Crusade I raided with Heresy on Windrunner on my Shaman named Shaxes until the guild disbanded at Kalygos in Sunwell. I then raided with Premonition on Windrunner until I was accepted into Forlorn Legacy (Windrunner) and moved on to doing Mu'ru and Kil'jaden kills PRE-NERF. After the expansion was over I raided with Forlorn Legacy (US17-20) untill TOC release. After TOC was released I raided with Incite of Manoroth, formally known as Misguided Angles of Mag'theradon (US 22). After becoming 12/12 25 HM ICC, I decided to main my priest and retired my resto shaman. I played throughout the rest of the expansion on my Priest and then join No Chicks Allowed on my priest during the first week of Cataclysm and then went casual. Now I just play casually, in Casual with some friends, who suggested I apped to you kids.
6. It is assumed you will be available from 8-12 ST (Server Time is EST) Tuesday through Thursday, during progression we will raid Sunday and Monday as well starting at 8 ST and ending when all encounters are defeated. If you can’t, please explain why you couldn't make our scheduled raid times (or why you even bothered applying):
I can make all of these. Work will permit!
7. Post a screen shot of your UI and/or a comprehensive list of mods that you use for raiding?

It's not me raiding, but its the general gist of it. Raiding add-ons not shown, Power Auras (can't live without it.) Deadly Boss Mods (who wouldn't have this?), Omen is not displayed as healing agro is ROFL. I could go into a ton of detail about non-needed pretty interface add-ons, but I won't bore you.
8. If you have thoroughly answered the following questions and you are confident you are what we are looking for, you have a good chance of being trialed, but we'd like to know why you want to join us? What made you choose us over the other guilds out there?
Typical question. A few friends in Casual told me I should feel you guys out. You are the 2nd most progressed 25 man alliance guild on the server and I am looking for a well established 25 man group to raid with. I need people who know what they're doing, and how to play well at all time. I need to raid with people who care about progression and how they can improve. I have a high standard when it comes to playability and I prefer to raid with members who are on the same level if not better to help motivate others to do well. And from what I have heard, you guys seem to mesh well with that minus the trolling.
But now for some addition information, I didn't include above. I know that I am a top US/World player, but I don't let that get to my head. I hope you find my experience the appetizing part of this application. I lack some gear on my priest, and a bunch on my druid, but gear is easy to come by, however good players, are not. I hope you take the chance to trial me, I don't have any intentions of losing motivation to raid or to be in your guild. Once I am committed, I won't back down or leave. Coming from such high guilds has giving me the ability to be level headed and most of all patient. I learn that kills come with time and that slow progression isn't the same as no progression. Once again, I hope you find me to be what you're looking for.
